Overview
This course highlights the definition and purpose of developing a care plan. It explains why care plans matter in supporting participant-centered services and in achieving positive health outcomes.
Objectives
By the end of this course, you should be able to:
- Define a care plan and understand its purpose in contributing to positive health outcomes for WIC participants.
- Understand the six-steps of a care plan to tailor and prioritize nutrition services to meet participants needs.
- Understand how to engage participants to identify challenges and strengths as well as protective factors.
Understand how to use open ended questions, reflective listening, and empathy to establish a realistic and meaningful goal with clear action steps and follow-up plans.
